I've had credit cards and loans in the past but no longer have the paper work so I do not know if I had PPI on them. Is there a template letter available to use to send to a bank for this scenario?

Yes I used moneysavingexpert -PPI templates. It is Martin Lewis's site and full of good info about reclaiming. Do NOT use any companies, do it yourself.
